• Directly from you
• From the Medical Group, Pharmacies, and our platform provider (order, prescription status, and shipping information only)
• Automatically from your device when you use our Services
• From service providers, such as payment processors

4. Consumer Health Data We Share and With Whom

We share the categories of consumer health data listed in Section 1 with the following:

• The Medical Group and Pharmacies, to provide the care and products you request
• Service providers (processors) that act on our behalf under contracts restricting their use of the data, such as hosting and platform providers, payment processors, shipping carriers, and customer support providers
• Our affiliates: We do not share consumer health data with affiliates.
• Government authorities or others, when required by law

We do not sell consumer health data.

5. Consent

• We collect consumer health data only with your consent or as necessary to provide a product or service you have requested. We share consumer health data only with your separate consent or as necessary to provide a product or service you have requested.
• You may withdraw your consent at any time by contacting us as described below.
• We do not use geofencing to identify or track consumers seeking health care services.

6. Your Rights

Subject to applicable law, you have the right to:

• confirm whether we collect, share, or sell your consumer health data, and access that data, including a list of all third parties and affiliates with whom we have shared it and a way to contact them;
• withdraw your consent to our collection or sharing of your consumer health data;
• request deletion of your consumer health data, which we will also pass on to our processors and to any third parties or affiliates with whom we shared it; and
• not be discriminated against for exercising these rights.

7. How to Exercise Your Rights

• If we deny your request, you may appeal by emailing info@tempomeds.com with the subject line “Health Data Appeal.” We will respond within 45 days. If your appeal is denied, you may contact the Washington State Attorney General at www.atg.wa.gov/file-complaint, the Nevada Attorney General at ag.nv.gov, or the attorney general of your state.
